By checking the waiver acceptance box, you acknowledge and agree to the following terms and conditions related to your participation in activities at Catch Jiu Jitsu:
Assumption of Risk:
You understand that participating in martial arts, including Jiu Jitsu, involves inherent risks of physical injury, including but not limited to sprains, fractures, concussions, and other serious injuries. You voluntarily assume all risks associated with your participation in any and all activities, training sessions, and events offered by Catch Jiu Jitsu.
Medical Condition:
You affirm that you are in good physical condition and do not suffer from any disability, illness, or condition that would prevent or limit your participation in the activities. You agree to consult with a medical professional if you have any doubts about your ability to participate safely.
Release and Waiver of Liability:
In consideration of being permitted to participate, you, for yourself, your heirs, executors, administrators, and assigns, hereby release, waive, discharge, and covenant not to sue Catch Jiu Jitsu, its instructors, employees, agents, and affiliates from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by you, or to any property belonging to you, whether caused by the negligence of Catch Jiu Jitsu or otherwise, while participating in any activity, or while in, on, or upon the premises where the activities are being conducted.
Indemnification:
You agree to indemnify and hold harmless Catch Jiu Jitsu, its instructors, employees, agents, and affiliates from any loss, liability, damage, or cost that may incur due to your participation in the activities, whether caused by negligence or otherwise.
Governing Law:
This waiver shall be governed by and construed in accordance with the laws of Taiwan, without regard to its conflict of law principles. Any legal action or proceeding arising out of or relating to this waiver shall be brought exclusively in the courts located in Kaohsiung City, Taiwan.
Emergency Medical Treatment:
In the event of an emergency, you authorize Catch Jiu Jitsu, its instructors, or agents to seek and consent to medical treatment for you as they deem necessary. You agree to be responsible for any medical expenses incurred.
Severability:
If any provision of this waiver is found to be unenforceable, the remaining provisions shall remain in full force and effect.
By proceeding with registration, you acknowledge that you have read this waiver and release of liability, understand its contents, and voluntarily agree to its terms.
